Eighteen months into implementation of the CMS GUIDE Model, public health partners are playing a critical role in shaping local success. This webinar will feature BOLD partners who have developed collaborative relationships with GUIDE programs within their local health systems, sharing how these partnerships were formed and sustained. Attendees will gain practical insights into effective collaboration strategies and lessons that can inform GUIDE implementation in their own communities.
