Course Description: While neither hospice nor palliative care are new concepts, there remains a great deal of confusion about the distinction between the two.  How are they the same, how are they different? Which option is right for your patient, and at what time in their serious illness journey should you consider a referral?  These questions (and more!), as well as how to talk to both patients and referral partners (physicians, discharge planners, etc.) about serious illness care options will be the focus of this engaging session.      

OBJECTIVES:  At the end of this session participants will be able to:

• List both the similarities and differences between palliative care and hospice care
• Articulate and dispel pervasive myths that are typically associated with palliative care or hospice
• Note the differences in eligibility criteria between palliative care or hospice.
• Understand the difference in regulations relative to reimbursement for palliative care or hospice
• Identify best practices in communication about serious illness applied to different audiences
