Course Description: While neither hospice nor palliative care are new concepts, there remains a great deal of confusion about the distinction between the two. How are they the same, how are they different? Which option is right for your patient, and at what time in their serious illness journey should you consider a referral? These questions (and more!), as well as how to talk to both patients and referral partners (physicians, discharge planners, etc.) about serious illness care options will be the focus of this engaging session.
